How to Register as a New Patient

You can register at our Practice if you qualify for NHS care and live within our Practice boundary, covering, St Mellons, Rumney, Llanrumney and Trowbridge.

Registration forms and other documentation can be collected at reception desk

Children under 16 can only register if their parent/guardian is registered at the Practice

If there is someone with whom you would like your doctor to be able to discuss your medical records, for example a close relative or friend, please complete the form below.

Third party consent form

If you have a sensory loss, whether visual or hearing, please let us know so that we can make sure that we provide services to you in an appropriate way. If you would like us to communicate with you in a different format, e.g. larger font size when we write to you, please let us know.
